• Resolution of Society, Trust or Company for becoming a Study Center (to be typed on Letter Head of study centre)
• Details of infrastructure(attach proof)
• Photograph of the Institution, Classrooms, Lab, Library, Reception & front view of the Institution. (Minimum photos required 5 copies)
• Bio data of Study Centre Co?ordinator with documents(Passport size photo, Passport, Voter Id card, Driving License, academic certificates)
• Bio Data of Faculty Members with copies of their educational certificates and contact details
• Computer, Printer, Scanner and Internet Connection
• A reliable Mobile No and Email Id of the Study Centre
• Signed copy of MOU with nearby recognized school for examination
• Self declaration Form (to be typed in Rs.100/- Indian Non-judicial Stamp paper) duly signed by Notary
• Inspection fee – a non refundable Fee Rs. 2000/-
• Recognition fee – a non refundable Fee Rs. 28000/-
• All documents must be duly attested with signature and institution seal.
